Description
Positioned along the anterolateral ankle, the superior extensor retinaculum spans from the distal fibula toward the distal tibia, forming a transverse fascial band just proximal to the talocrural joint line. Deep to it, the extensor tendons would course anterior to the lateral malleolus and over the talar neck region, held close to the anterior surface of the distal tibia and fibula. Lateral bony landmarks include the fibular shaft, lateral malleolus, talus, and calcaneus, with the midfoot (cuboid and navicular) and forefoot elements receding distally. Blue ligamentous structures around the ankle mortise frame the retinacular attachment zone. Clinically, the superior extensor retinaculum matters because it prevents bowstringing of tibialis anterior, extensor hallucis longus, and extensor digitorum longus during dorsiflexion, and it defines a tight corridor for the deep fibular (peroneal) nerve and anterior tibial vessels as they cross the ankle. Small changes in retinacular tension after ankle sprain, scarring, or postsurgical swelling can contribute to anterior ankle pain or neurovascular irritation, a pattern often discussed alongside anterior ankle impingement and post-traumatic synovitis. A practical surface landmark. The lateral projection also helps clarify the relationship between retinaculum, distal fibula, and the anterior margin of the ankle mortise.
