Across the anterior aspect of the distal crus, the superior extensor retinaculum spans transversely from the distal fibula laterally to the anterior margin of the distal tibia medially, forming a thickened fascial band just proximal to the talocrural joint line. Deep to it, the anterior tendons that cross the ankle would be expected in close apposition to the tibia and talus, including tibialis anterior, extensor hallucis longus, and extensor digitorum longus as they course distally toward the dorsum of the foot. The tibial plafond and fibular malleolar region frame the superior talus inferiorly, with articular cartilage highlighted over the ankle mortise and the talar trochlea; the calcaneus sits posteroinferiorly, and the metatarsals and phalanges extend distally.
