Description
Shown from the lateral aspect of the head and upper neck, the temporomandibular joint (TMJ) capsule encloses the articulation between the mandibular condyle and the mandibular fossa of the temporal bone, with the articular eminence positioned anterior to the condylar head. The capsular ligament spans from the rim of the temporal articular surface to the neck of the mandible, and the synovial membrane is implied as the inner lining of this fibrous envelope. Surrounding bony landmarks provide orientation, including the zygomatic bone and zygomatic arch laterally, the mandibular ramus inferiorly, and the cranial vault with coronal, sagittal, and lambdoid sutures superior and posterior. Cervical vertebrae C1 to C7 align inferior to the occiput in profile. A useful landmark. TMJ pain is rarely “just a joint” in practice, so a strict lateral view of the capsule matters when teaching where symptoms localize and where interventions land. Capsulitis and synovitis can follow bruxism or prolonged mouth opening, and patients often report preauricular pain that worsens with translation as the condyle moves anteriorly toward the articular eminence. For procedural planning, appreciating the capsule’s attachment to the condylar neck helps explain the target zone for arthrocentesis and the risk of entering adjacent spaces when needle trajectory is too posterior, close to the external acoustic meatus and retrodiscal tissues. Dental anatomy, orofacial pain, and head and neck courses can use this asset to ground TMJ biomechanics before introducing the articular disc, lateral pterygoid influence, and common imaging workups such as MRI for internal derangement. It also fits surgical atlases covering preauricular approaches, arthroscopy portals, and capsule-preserving dissection around the zygomatic arch and mandibular ramus.
