Description

Running inferiorly from the jugular foramen, the cervical vagus nerve (cranial nerve X) descends within the carotid sheath, positioned posteriorly between the internal jugular vein laterally and the internal or common carotid artery medially. Pharyngeal branches course anteromedially toward the pharyngeal plexus along the middle pharyngeal constrictor, while the superior laryngeal nerve arises high in the neck and divides into the internal branch piercing the thyrohyoid membrane and the external branch tracking inferomedially toward the cricothyroid muscle. Inferiorly, the recurrent laryngeal nerve is represented ascending in the tracheoesophageal groove to the larynx, emphasizing the vagus contribution to laryngeal and pharyngeal innervation and parasympathetic autonomic supply to thoracic viscera. Key landmark relationships are maintained. For surgeons and trainees, a lateral perspective makes the vagus nerve’s deep course easier to teach because it clarifies why carotid endarterectomy, anterior cervical spine exposure, and thyroidectomy place nearby branches at risk even when the main trunk is not directly handled. Injury to the recurrent laryngeal nerve produces hoarseness and impaired vocal fold abduction, while disruption of the external branch of the superior laryngeal nerve weakens pitch control via cricothyroid paresis.
