Description
Arising from the lateral medulla at the postolivary sulcus, the vagus nerve (cranial nerve X) descends within the carotid sheath, positioned posteriorly between the common carotid artery medially and the internal jugular vein laterally. From this lateral perspective in a male adult, its cervical course can be followed inferiorly toward the thoracic inlet, where it continues into the superior mediastinum alongside the great vessels. Bony landmarks frame the field, with the rib cage and thoracic vertebral column posteriorly and the scapula partially visible superiorly. Small branches are implied along its descent, consistent with superior and inferior vagal (nodose) ganglia in the upper neck and early cardiopulmonary branching as it approaches the thorax. Clear relationships. Tight spacing. That carotid sheath relationship is the point of the view, because it is where iatrogenic vagus injury and traction occur during carotid endarterectomy, anterior cervical exposure, and central venous access via the internal jugular vein. Clinically, it also explains why vagal stimulation or irritation in the neck can produce bradycardia, cough, or hoarseness through downstream effects on cardiac plexus input and recurrent laryngeal nerve function as CN X transitions into the thorax. A lateral angle makes the nerve’s posterior position relative to the artery and vein easier to teach than an anterior view.
