Description
Arising from the lateral medulla oblongata as cranial nerve X, the vagus nerve descends within the carotid sheath of the neck, positioned posterolateral to the common and internal carotid arteries and closely related to the internal jugular vein. In the superior mediastinum it tracks posterior to the roots of the lungs, giving a recurrent laryngeal branch that loops on the right under the subclavian artery and on the left under the arch of the aorta adjacent to the ligamentum arteriosum, then ascends in the tracheoesophageal groove. Inferiorly, vagal fibers contribute to the pulmonary plexus and esophageal plexus before reorganizing into anterior and posterior vagal trunks near the esophageal hiatus. Long course, small caliber. Clinically, an anterior-oriented rendering of the vagus is the cleanest way to teach its shifting relationships as it transitions from carotid-space anatomy to mediastinal great vessels and finally to the esophagus. Left recurrent laryngeal nerve vulnerability in aortic arch pathology and during patent ductus arteriosus ligation is a classic correlation, while the right recurrent laryngeal nerve is more often encountered in thyroid and paratracheal surgery. You can also map the expected pattern of hoarseness from recurrent laryngeal injury and contrast it with dysphagia when the esophageal plexus is disrupted. Use this asset in head and neck anatomy lectures, cardiothoracic surgical atlases (aortic arch, pulmonary hilum, esophageal mobilization), and endocrine surgery teaching modules that need a reliable cranial nerve X pathway from brainstem to thoracoabdominal transition. It also fits well in clinical handouts explaining vocal fold paresis after mediastinal tumor compression or post-thyroidectomy neuropraxia.
