Description
Viewed from the left lateral aspect, the left ventricle dominates the silhouette, with the left atrial appendage positioned anterosuperiorly near the root of the pulmonary trunk. The left coronary artery is represented as it emerges from the left aortic sinus and quickly divides into the anterior interventricular branch (left anterior descending artery) descending toward the apex in the anterior interventricular sulcus and the circumflex branch coursing posterolaterally within the coronary (atrioventricular) sulcus. Alongside the arterial conduit, the great cardiac vein parallels the anterior interventricular artery, then turns into the left atrioventricular groove to continue as the coronary sinus on the posterior surface. Red arterial vasculature contrasts cleanly with blue venous return. Left-sided coronary anatomy is the workhorse view for understanding perfusion territories implicated in anterior and lateral myocardial infarction, where occlusion of the LAD or obtuse marginal branches off the circumflex can produce characteristic ECG patterns and regional wall-motion abnormalities. The relationship of the circumflex artery to the left atrium and mitral annulus is also clinically tight, a key consideration during mitral valve surgery and in coronary sinus based electrophysiology procedures. Coronary dominance and the variable size of the circumflex system can be introduced from this perspective without losing the course of the LAD to the apex. Clear landmarks. Use this asset in gross anatomy and cardiovascular blocks to teach the coronary sulci, vessel naming conventions, and venous drainage pathways, and in cardiology or cardiothoracic surgery materials that discuss STEMI culprit lesions, bypass graft targets (LIMA to LAD, saphenous vein to obtuse marginal), and catheter-based mapping near the coronary sinus.
