Description
Oriented from the medial aspect, the spleen is presented with the visceral surface facing the viewer, bringing the splenic hilum (hilum splenicum) and its vascular pedicle into clear relief. The splenic artery (a. splenica), shown in red, approaches the organ and divides into segmental branches at the hilum, while the splenic vein (v. splenica), in blue, forms a converging pattern as tributaries join and exit medially. Fine-caliber vessels consistent with trabecular arteries and veins can be inferred extending into the parenchyma from the hilar region. The capsule remains smooth, with the concavity of the hilum interrupting the otherwise convex contour. Medial visualization of the splenic vasculature matters because most operative and interventional decisions hinge on the hilar branching pattern, not the diaphragmatic surface anatomy seen from the lateral side. Segmental arterial division at or near the hilum explains why partial splenectomy and spleen-preserving approaches can be planned along relatively discrete vascular territories, while also highlighting where hilar control becomes hazardous in splenomegaly or post-traumatic distortion. Short gastric vessels are not the focus here, so attention stays on the main inflow and outflow. This is the bleeding axis.
