Description
Arising from the distal femoral artery within the adductor canal, the descending genicular artery courses distally and medially toward the knee, passing anterior to the medial femoral epicondyle and then dividing toward the medial joint line. Along its path, the vessel runs in close relationship to the sartorius and vastus medialis, with the saphenous nerve typically traveling nearby as it becomes superficial on the medial side of the knee. Branches contribute to the periarticular genicular anastomosis, with accompanying veins tracking parallel to the artery and draining proximally. Articular cartilage caps the medial femoral condyle and medial tibial plateau deep to the vascular plane. Medial exposure of the descending genicular artery matters because it is a dependable landmark when planning flap perfusion around the knee and when discussing collateral circulation after superficial femoral artery occlusion. Its saphenous (superficial) branch is frequently used as the pedicle for a descending genicular artery flap to cover defects around the patella, proximal tibia, and medial knee after trauma or tumor resection, so the artery’s relationship to the medial epicondyle and joint capsule is not academic. A small vessel. Yet surgeons care about it.
