Description
Arising from the medial process of the calcaneal tuberosity, the flexor digitorum brevis lies in the second layer of the plantar foot, deep to the plantar aponeurosis and superficial to quadratus plantae and the tendons of flexor digitorum longus. From a medial perspective, its fleshy belly spans the central sole, tapering distally into four tendons that pass anteriorly to the toes and split around the flexor digitorum longus tendons before inserting on the sides of the middle phalanges of digits 2 through 5. Medially, the muscle sits lateral to the medial longitudinal arch and adjacent to the first ray, while the calcaneus, talus, and midfoot tarsals provide the posterior bony backdrop. That medial oblique view matters because it clarifies the layered relationship of the intrinsic plantar flexors, the course of the digital tendons at the metatarsophalangeal and proximal interphalangeal levels, and the way the flexor digitorum brevis supports toe purchase during stance. It also helps orient the clinician to the region explored in plantar fasciitis and plantar heel pain workups, where tension in the plantar aponeurosis and nearby intrinsic musculature is assessed, and in medial approaches that must respect the adjacent neurovascular plane of the medial plantar nerve as it courses between the abductor hallucis and flexor digitorum brevis.
