Description
Medial exposure of the lower leg centers on the flexor digitorum longus muscle (FDL) as it arises from the posterior surface of the tibia, inferior to the soleal line, with its muscle belly positioned posteromedial to the tibial shaft. Distally, the FDL tendon courses deep in the posterior compartment toward the ankle, passing posterior to the medial malleolus beneath the flexor retinaculum, then continuing into the sole to divide into four slips for the lateral four toes. Tibialis posterior lies adjacent and usually more anterior at the medial ankle, while flexor hallucis longus approaches from the posterolateral side and crosses deep in the plantar midfoot at the master knot of Henry. Orientation is clear. This medial perspective matters because it tracks the FDL through the same confined fibro-osseous tunnel that also contains tibialis posterior, flexor hallucis longus, the posterior tibial artery and veins, and the tibial nerve, making it a practical teaching view for tarsal tunnel syndrome and medial ankle pain. When clinicians evaluate flexible clawing of the lesser toes or dysfunction after tibial fractures, understanding the FDL’s long tibial origin and its retromalleolar course helps explain traction, adhesions, or tenosynovitis along the tendon sheath. Surgeons also reference this corridor during posteromedial ankle approaches and tendon transfers, where protecting the tibial neurovascular bundle and distinguishing FDL from tibialis posterior is nonnegotiable.
