Description
Seen from the medial aspect of the male lower limb, the flexor hallucis longus is highlighted in the deep posterior compartment, with its muscle belly arising along the posterior fibula and adjacent interosseous membrane, lying lateral to the tibialis posterior and posterior to the distal tibia. Distally, the tendon courses posterior to the medial malleolus within the tarsal tunnel, running deep to the flexor retinaculum and crossing the plantar foot toward the hallux. Superficial landmarks such as gastrocnemius and soleus frame the deeper anatomy, while the tibia, fibula, talus, calcaneus, and the medial column of the foot provide bony orientation. This medial view matters because it tracks the flexor hallucis longus tendon through the same confined space that houses the posterior tibial tendon, flexor digitorum longus, and tibial neurovascular bundle, a setup that explains posteromedial ankle pain in tarsal tunnel syndrome and stenosing tenosynovitis.
