Description
Posteriorly, the deep posterior compartment of the male leg is opened to isolate the flexor hallucis longus (FHL) along the posterior fibula, with the fibula lateral and the tibia medial in anatomical position. The muscle belly lies inferior to the knee region and tapers distally into a tendon that tracks toward the posteromedial ankle, while tibialis posterior sits more medial and closer to the posterior tibial surface. Between these planes you would expect the posterior tibial neurovascular bundle to course distally, with FHL forming a lateral boundary in the lower leg as it approaches the tarsal tunnel region. FHL anatomy matters because its tendon passes posterior to the talus and under the sustentaculum tali before reaching the hallux, making it a frequent pain generator in posterior ankle impingement, tenosynovitis, and os trigonum syndrome seen in dancers and running athletes. Surgical orientation also depends on this relationship: tibialis posterior and flexor digitorum longus lie medial to FHL at the ankle, and confusing these tendons can compromise tendon transfer planning or endoscopic release procedures around the posteromedial ankle.
