Posterior lower leg anatomy is presented with the tibia positioned medially and the fibula laterally, framing the deep posterior compartment. Flexor hallucis longus is highlighted along the posterior surface of the fibula, its muscle belly lying deep to soleus and tapering distally into a tendon that courses behind the medial malleolus toward the hallux. Proximally, the relationship to the knee region is oriented by the posterior aspects of the tibiofibular region, while distally the muscle-tendon unit aligns with the ankle and hindfoot.
