Description
Seen from the medial aspect of the male leg, the tibialis posterior occupies the deep posterior compartment, lying posterior to the interosseous membrane and tucked between the medial surface of the fibula and the posterior surface of the tibia. Its muscle belly sits inferior to the popliteus region and deep to the soleus, while the long tendon narrows distally and passes posterior to the medial malleolus, where it turns plantarward toward its insertions on the navicular tuberosity and adjacent tarsals. Nearby deep compartment companions such as flexor digitorum longus (medial) and flexor hallucis longus (more lateral, fibular) are logically oriented around it, with the tibial neurovascular bundle running close in the same fascial plane. Deep. Central. Medial exposure matters because the tibialis posterior tendon is a primary dynamic support of the medial longitudinal arch, and its relationship to the medial malleolus explains both its function as an inverter and the mechanics of posterior tibial tendon dysfunction in adult acquired flatfoot. The course behind the medial malleolus is also the same corridor used when assessing the tarsal tunnel and when planning tendon debridement, reconstruction, or transfer procedures, where distinguishing tibialis posterior from flexor digitorum longus is non-negotiable. Palpation and ultrasound landmarks follow the same anatomy.
