Description
Arising from the posterior surfaces of the tibia and fibula and the intervening interosseous membrane, the tibialis posterior is presented as a deep posterior compartment mass tucked anterior to the soleus and gastrocnemius and medial to the fibula. Its tendon narrows as it courses distally behind the medial malleolus, passing deep to the flexor retinaculum in close company with the flexor digitorum longus and flexor hallucis longus tendons. Relative to the sural (triceps surae) musculature, tibialis posterior sits deep and slightly medial, with its line of pull directed toward the plantar-medial midfoot. Deep. Hidden. Clinically consequential. Posterior and slightly lateral viewing makes the relationships that matter in the tarsal tunnel easier to teach: tibialis posterior runs immediately posterior to the tibia, and the tendon’s sharp turn around the medial malleolus explains both its mechanical advantage for inversion and its vulnerability to attritional tenosynovitis. Posterior tibial tendon dysfunction is a common driver of adult-acquired flatfoot, and the deep location beneath the superficial calf muscles helps explain why early disease can be missed on routine inspection. Surgical planning also hinges on these planes, from tendon debridement and repair to transfers that aim to restore hindfoot inversion and support the medial longitudinal arch.
