Description

Positioned along the anterolateral surface of the tibia, the tibialis anterior muscle belly occupies the proximal and mid leg and tapers distally into a tendon that crosses the anterior aspect of the ankle joint. The tibial shaft forms the medial border of the field, while the fibula lies lateral and slightly posterior, creating the interosseous space that separates the anterior and lateral compartments. Distally, the tendon runs inferior to the extensor retinacula on its way toward the medial foot. Clear bony landmarks anchor the orientation. Clinically, the tibialis anterior is the primary dorsiflexor during swing phase, and weakness produces the familiar steppage gait of peroneal neuropathy or L4 to L5 radiculopathy. The anterior course of its tendon at the ankle explains why it is palpated just lateral to the tibial crest and why anterior compartment syndrome threatens its function early, with pain on passive plantarflexion and risk to the deep fibular (peroneal) nerve.
