Description
Running deep in the posterior compartment of the male leg, the tibialis posterior is visualized beneath the soleal line on the posterior tibia, with the fibula lying lateral to it and helping define the interosseous space. Medial and slightly anterior to the flexor hallucis longus, its muscle belly tapers into a tendon that descends distally toward the medial malleolus. The flexor digitorum longus typically lies more medially, creating the familiar deep posterior “TP-FDL-FHL” arrangement as the tendons approach the ankle. Deep, compact, and easy to miss. Clinically, tibialis posterior matters because its tendon is the primary dynamic support of the medial longitudinal arch, and degeneration at or just distal to the medial malleolus drives posterior tibial tendon dysfunction with progressive adult-acquired flatfoot. This view clarifies why the tendon is vulnerable as it turns around the malleolus within the tarsal tunnel region, and it helps differentiate tibialis posterior pathology from flexor hallucis longus tenosynovitis, which is more common in dancers and runners and tracks toward the hallux. Surgeons also rely on these relationships during tendon transfer procedures and when releasing the flexor retinaculum while protecting the posterior tibial neurovascular bundle.
