Seen from a plantar angle, the tibialis posterior tendon courses distally behind the medial malleolus and fans out across the plantar-medial midfoot, with slips directed toward the navicular tuberosity and the medial cuneiform, then extending laterally to the intermediate and lateral cuneiforms, the cuboid, and the bases of the second to fourth metatarsals. Its division lies deep to the flexor retinaculum proximally and then passes superficial to the plantar ligaments at the naviculocuneiform region, where it interlaces with the spring (plantar calcaneonavicular) ligament and adjacent capsular fibers. Around it, the plantar tarsal bones and metatarsals provide clear landmarks for orientation, including the talus superiorly and the calcaneus posteriorly. Multiple insertions are the point. That branching pattern is the mechanical explanation for why posterior tibial tendon dysfunction produces a progressive collapse of the medial longitudinal arch and forefoot abduction: when the tendon frays or elongates near the navicular insertion, the hindfoot tends toward valgus and the talonavicular joint loses restraint, even if the Achilles and peroneal tendons remain intact. A plantar perspective also supports teaching of the tarsal tunnel neighborhood, since the tendon runs anteromedial to the calcaneus and adjacent to the flexor digitorum longus and flexor hallucis longus tendons as they turn toward the sole. Surgeons plan debridement, tendon transfer, or spring-ligament reconstruction by thinking in these exact relationships.
