Description
Plantarly oriented anatomy of the male foot centers on the tibialis posterior tendon as it courses from the posteromedial ankle into the sole, lying deep to the plantar aponeurosis and long plantar ligament and approaching the medial longitudinal arch. The tendon is shown inserting on the navicular tuberosity and sending slips distally toward the medial and lateral cuneiforms, cuboid, and bases of the metatarsals, with the talus and calcaneus positioned proximally and the tarsometatarsal complex arranged inferiorly toward the toes. Medially, the tendon runs adjacent to the plantar calcaneonavicular (spring) ligament under the head of the talus, a tight spatial relationship that explains how tendon failure destabilizes the arch. Tibialis posterior tendon dysfunction is a leading cause of adult acquired flatfoot, and a plantar view makes its mechanical role tangible: loss of tension permits plantarflexion and adduction collapse at the talonavicular joint, with secondary attenuation of the spring ligament and progressive hindfoot valgus. Pain often localizes along the posteromedial ankle, but the deformity expresses itself in the sole as widening of the midfoot and uncovering at the talar head.
