Description

Running obliquely across the posterior proximal forearm, the supinator wraps from the lateral epicondyle of the humerus and the crest of the ulna to encircle the neck of the radius, its fibers spiraling to insert on the lateral and anterior surfaces of the proximal radial shaft. The muscle sits deep to the extensor compartment, occupying the radial side of the posterior elbow just distal to the humeroradial joint. Medially, the ulna forms the stable axis, while the radius rotates laterally around it during supination at the proximal radioulnar joint. Clear appreciation of this posterior view helps when teaching the mechanics of forearm rotation, because the supinator’s helical fiber direction explains how it produces supination without elbow flexion, unlike the biceps brachii. It also frames a common clinical problem: the deep branch of the radial nerve (posterior interosseous nerve) passes through the supinator at the arcade of Frohse, a frequent site of entrapment producing finger and thumb extension weakness with minimal sensory loss. Small muscle. Big consequence.
