Description
Curving around the proximal third of the radius, the supinator forms a deep sleeve on the posterolateral forearm that becomes visible from an anterior aspect as it wraps from the lateral epicondyle and supinator crest of the ulna toward the lateral surface of the radial shaft. Proximally, its superficial fibers arise near the lateral epicondyle beneath brachioradialis and extensor carpi radialis longus and brevis, while deeper fibers encircle the radius just distal to the radial head and neck. Medial to the muscle belly sit the ulna and the proximal radioulnar joint, with the radius positioned laterally in an anatomic position forearm. Function drives the interest here: supinator produces forearm supination without elbow flexion, a distinction that becomes obvious when compared with biceps brachii, which supinates most powerfully when the elbow is flexed. A key clinical relationship runs through its proximal fibers, where the deep branch of the radial nerve passes between the two heads (Arcade of Frohse) to become the posterior interosseous nerve, a common entrapment site causing finger and thumb extension weakness without sensory loss. That nerve course also frames the proximal surgical anatomy during approaches to the radial head and neck.
