Description
Medial ankle anatomy is opened to the flexor retinaculum (laciniate ligament), a dense fibrous band spanning from the posterior border of the medial malleolus to the medial calcaneus and blending inferiorly with the deep fascia of the foot. Deep to this roof, the tarsal tunnel contents course posterior to the tibia in classic order: tibialis posterior tendon most anterior, then flexor digitorum longus, the posterior tibial artery with paired venae comitantes, the tibial nerve, and flexor hallucis longus most posterior as it approaches the sustentaculum tali. The calcaneal (Achilles) tendon remains posterior and superficial, while the talus, calcaneus, and navicular define the medial hindfoot skeleton and the longitudinal arch contour. Clinically, this medial perspective clarifies the biomechanics and compression points of the tarsal tunnel, where tibial nerve entrapment can produce plantar paresthesia and medial heel pain that often mimics plantar fasciitis. Surgical release hinges on recognizing the retinaculum’s attachments and the septations that can compartmentalize the tunnel, especially around the neurovascular bundle at the level of the medial malleolus and the sustentaculum tali.
