Centered at the volar wrist, the transverse carpal ligament (flexor retinaculum) spans from the radial carpal eminence (scaphoid tubercle and trapezium crest) to the ulnar carpal eminence (pisiform and hook of hamate), forming a dense fascial band superficial to the carpal bones. Proximal forearm flexor muscle bellies transition into long tendons that pass distally toward the digits, while the palmar aponeurosis and superficial fascia sit anterior to the retinaculum. Medially, the ulnar side of the wrist is organized around the pisiform region, and laterally the thenar side frames the radial border of the carpal tunnel. Blue neurovascular elements course longitudinally along the volar aspect, positioned relative to the tendon bundles and retinacular margins. An anterior perspective is the teaching view for the carpal tunnel as an osteofibrous canal: carpal bones posteriorly, transverse carpal ligament anteriorly, and the flexor digitorum superficialis and profundus tendons plus the flexor pollicis longus tendon traveling within, with the median nerve vulnerable to compression. This is where classic carpal tunnel syndrome lives, and the spatial relationship between the retinaculum and tendon mass explains nocturnal paresthesias and why provocative wrist flexion worsens symptoms.
