Arising from the dorsalis pedis artery on the dorsum of the foot, the dorsalis metatarsal arteries course distally over the metatarsals toward the bases of the toes, with the first dorsalis metatarsal artery running closest to the hallux on the medial side. From a medial perspective, the arterial tree can be followed as it passes anterior to the ankle joint and continues along the dorsal midfoot, while paired dorsal digital branches diverge toward adjacent sides of the toes. Superficial veins accompany this pathway in parallel, returning blood proximally along the dorsum toward the ankle. Small vessels. Clear landmarks. Medial visualization matters because the first web space and great toe are frequent sites for vascular assessment and surgical planning, and the caliber and course of the first dorsalis metatarsal artery often determine flap design in dorsal metatarsal artery perforator procedures and in reconstructions requiring a dependable dorsal inflow. The same segment is commonly interrogated in diabetic foot evaluation and peripheral arterial disease, where diminished dorsal flow can correlate with ischemic changes at the hallux and distal forefoot.
