Description
Running distally across the plantar aspect of the forefoot, the plantar metatarsal arteries arise from the deep plantar arch and course in the intermetatarsal spaces, deep to the flexor tendons and adjacent to the metatarsal shafts. Each vessel tracks anteriorly toward the web spaces, where it contributes to the common plantar digital arteries supplying the toes, while venous counterparts accompany them along the same planes. Intrinsic muscles of the central compartment, including flexor digitorum brevis and quadratus plantae proximally, and the lumbricals and interossei more distally, frame the neurovascular bundles between the metatarsals. Proximal phalanges, metatarsals, and the plantar aponeurosis form the bony and fascial backdrop. For clinicians, this plantarly oriented dissection-style view clarifies why the plantar metatarsal arteries are at risk during forefoot surgery and deep plantar injections, where a few millimeters of depth can move you from tendon sheath to neurovascular bundle. It also supports teaching around the vascular basis of toe perfusion and wound healing, including why ischemic ulcers in diabetes and peripheral arterial disease often declare themselves in the distal forefoot and interdigital regions rather than over the midfoot. Small vessels matter here.
