Description
Running in the plantar forefoot, the plantar metatarsal arteries arise from the deep plantar arch and course distally in the intermetatarsal spaces between the metatarsal bones, just deep to the plantar interossei and alongside the corresponding metatarsal veins. Proximally, inflow comes through the posterior tibial artery and lateral plantar artery, while venous drainage tracks back via paired venae comitantes toward the tibial venous system. In the distal foot these vessels continue toward the toes as common plantar digital arteries that divide into proper plantar digital arteries. Small, orderly vessels. Clear trajectories. For clinicians, the plantar metatarsal arteries matter because they define the perfusion of the metatarsal heads and plantar soft tissues where pressure peaks during gait, and they are commonly encountered during plantar approaches for Morton neuroma excision, metatarsal osteotomy, and intermetatarsal space release. Their relationship to the deep transverse metatarsal ligament and the intermetatarsal neurovascular bundle explains why small variations in vessel position can turn a “simple” web-space dissection into brisk bleeding. Vascular compromise at this level also frames diabetic foot ulcer risk and forefoot wound-healing, where distal arterial disease can spare the dorsalis pedis yet leave plantar inflow inadequate.
