Description
Running along the posteromedial calf, the soleus forms a broad, flat muscle belly deep to the medial and lateral heads of the gastrocnemius, then narrows distally to join the calcaneal (Achilles) tendon. From a medial perspective, its proximal attachments relate to the posterior tibia along the soleal line and to the fibula via the tendinous arch, with the tibia lying medial to the fibula and the ankle mortise positioned inferiorly. Adjacent compartments are implied by the contour of tibialis posterior and the long flexors coursing posterior to the medial malleolus. Orientation is clear. Clinically, this view helps separate superficial gastrocnemius pathology from deep soleus problems, a distinction that matters in exertional calf pain and in rehabilitation planning after Achilles tendinopathy. The soleus is also a common site of deep venous thrombosis and of myofascial trigger points that refer pain to the heel, so its deep position and distal convergence on the calcaneus are practical landmarks when correlating with duplex ultrasound, MRI, or palpation. For surgeons and proceduralists, the medial calf corridor is relevant during posterior compartment fasciotomy and when considering the relationship of the soleus to the neurovascular bundle that passes deep to the soleal arch.
