Description
Sustentaculum tali projects medially from the calcaneus as a stout, shelf-like process positioned inferior to the talar body and medial to the posterior facet of the subtalar joint. From this fixed medial perspective, the medial wall of the calcaneus and the plantar aspect of the sustentaculum are emphasized, including the shallow sulcus for the flexor hallucis longus tendon on its inferior surface. The relationship to the talus is immediate: the sustentaculum underpins the middle talar facet and sits just posterior to the navicular direction, orienting the viewer to the medial longitudinal arch. Clinically, this bony ledge is a key landmark in hindfoot surgery and in teaching the subtalar joint complex because it anchors the spring ligament (plantar calcaneonavicular ligament) and supports the talar head during weight bearing. It matters in tarsal tunnel anatomy as well, since the neurovascular bundle runs posterior and inferior to the sustentaculum in the retinacular tunnel, and osteophytes or malunited fractures around this region can contribute to tibial nerve irritation. Fractures involving the sustentaculum are often relatively stable due to strong ligamentous attachments, yet they can alter subtalar congruity and the flexor hallucis longus tendon groove. Small structure, big consequences.
