Description
Posteriorly, the spinalis thoracis is highlighted as paired longitudinal muscle columns immediately adjacent to the thoracic spinous processes, forming the most medial component of the erector spinae group. Its segmented fascicles track superior to inferior across the posterior thorax, deep to trapezius and latissimus dorsi in situ, and closely paralleling the vertebral column from the upper thoracic region toward the thoracolumbar junction. Surrounding landmarks include the occiput and cervical spine superiorly, the scapulae over the posterior rib cage laterally, and the sacrum, iliac crests, and posterior pelvis inferiorly. Long bones of the upper and lower limbs frame the axial skeleton in anatomical position. Because spinalis thoracis sits so near the midline, this posterior full body perspective clarifies how extensor force is applied directly to the spinous processes to resist thoracic kyphosis and control segmental extension during lifting and prone trunk raising. Clinically, focal tenderness or myofascial trigger points near the thoracic spinous processes are often labeled as paraspinal strain, and this view helps separate a true midline spinalis pain pattern from more lateral longissimus involvement. It also supports interpretation of posterior approaches where thoracic paraspinal dissection and retraction can denervate or devascularize the medial erector spinae, contributing to postoperative paraspinal atrophy.
