Seen from the posterior aspect under intact skin, the paraspinal column is organized around the erector spinae mass, with spinalis thoracis forming the most medial vertical tract immediately adjacent to the thoracic spinous processes. Laterally, spinalis blends into longissimus thoracis, while iliocostalis occupies the outermost position toward the rib angles; together they create the characteristic paired muscle gutters that flank the vertebral column. Superiorly, the superficial trapezius spans from the nuchal region toward the acromion and scapular spine, and inferolaterally the latissimus dorsi sweeps over the thoracolumbar fascia toward its humeral insertion, framing the deeper extensor compartment. Distally, the posterior contour continues into the gluteal region and the posterior thigh and leg musculature. Spinalis thoracis often gets overlooked in favor of larger superficial muscles, but its medial position makes it a key landmark when teaching layered back anatomy and when correlating surface form with the underlying spinous processes. Palpation-based exams for thoracic paraspinal strain, facet-mediated pain patterns, and trigger points frequently localize pain to this medial gutter, while procedures such as thoracic medial branch blocks or paraspinal injections require a clear mental map of the midline versus paramedian muscular planes. Posterior approaches to the thoracic spine also traverse these extensor layers, so understanding where spinalis sits relative to longissimus and the spinous processes helps explain typical retraction planes and postoperative muscle morbidity.
