Description
Spinalis thoracis appears as a narrow, longitudinal column of muscle immediately adjacent to the thoracic spinous processes, forming the most medial component of the erector spinae in the posterior thorax. Superficial layers include the trapezius spanning from the cervical region to the mid thoracic spine, with rhomboid major and minor deep to it and coursing from the vertebral column to the medial border of the scapula. Inferior and lateral to these, latissimus dorsi sheets across the lower thoracic and lumbar back, while the bulk of erector spinae lies lateral to spinalis as longer fascicles (longissimus thoracis) and more lateral slips (iliocostalis). Fiber direction is emphasized to clarify the longitudinal line of pull relative to the vertebral column. Clear medial anatomy. Spinalis thoracis matters because its close apposition to the spinous processes makes it a reliable landmark when teaching the paraspinal compartment and when planning posterior midline approaches to the thoracic spine. In thoracic spine surgery, paraspinal muscle dissection and retraction correlate with postoperative pain and extensor weakness, and appreciating the medial spinalis plane helps explain why muscle sparing techniques and paramedian (Wiltse) intervals aim to reduce iatrogenic injury. The view also supports interpretation of axial MRI, where fatty infiltration or atrophy of the medial erector spinae can be discussed in chronic back pain, spondylosis, or after thoracolumbar instrumentation. Anatomy faculty can drop this into thoracic wall and back labs to differentiate trapezius, rhomboids, and the erector spinae columns, or into surgical teaching files for spine exposure, paraspinal compartment orientation, and rehabilitation discussions after posterior approaches.
