Description
Running longitudinally along the posterior thoracic spine, the spinalis thoracis forms the most medial column of the erector spinae, hugging the spinous processes from the upper lumbar region toward the mid to upper thoracic vertebrae. Lateral to it lies longissimus thoracis, while iliocostalis occupies the more posterolateral rib angles; together they create the paired paraspinal muscle masses flanking the vertebral midline. Superficial layers such as trapezius and latissimus dorsi are visible in grayscale, with the highlighted erector compartment standing out against the scapular region and posterior shoulder contour. The slight rightward turn clarifies the medial border of the paraspinal gutter and the symmetry of the thoracic spinous process line. Clinically, isolating spinalis thoracis helps when teaching segmental extension mechanics and the midline force couple that stabilizes thoracic posture during lifting, gait, and sustained sitting. Pain generators in this region are often labeled generically as paraspinal strain, but palpation and trigger point referral commonly localize to the medial erector mass adjacent to the spinous processes, where thoracic facet irritation and costovertebral stiffness also coexist. It matters in surgery, too, because midline posterior approaches to the thoracic spine depend on splitting or retracting these fibers off the spinous processes and laminae while preserving as much extensor function as possible. Midline anatomy is unforgiving.
