Description
Running in a narrow column immediately adjacent to the thoracic spinous processes, the spinalis thoracis is presented as the most medial component of the erector spinae mass in a posterior male back. Superficial layers under the skin include the trapezius over the upper thorax and neck and the latissimus dorsi sweeping inferolaterally toward the posterior axillary fold, while the longer iliocostalis and longissimus lie lateral to spinalis. Segmental attachments align with the midline bony landmarks, with fibers coursing superiorly across multiple thoracic vertebral levels. Orientation matters. Spinalis thoracis is often hard to separate from longissimus thoracis in dissection and on imaging because their fascial planes blend, so a clean posterior view keyed to the spinous processes helps you teach the medial-to-lateral organization of the erector spinae. Clinically, this region is a common source of paraspinal pain and a target for thoracic erector spinae plane blocks, where needle placement is judged relative to the thoracic transverse processes and the overlying paraspinal muscle envelope. Midline proximity is the point. Use this plate for gross anatomy labs when introducing intrinsic back muscles beneath trapezius and latissimus, or for rehabilitation and sports medicine materials explaining thoracic extension control and postural endurance.
