Description
Running longitudinally along the posterior thoracic spine, the spinalis thoracis occupies the most medial column of the erector spinae, hugging the spinous processes and sitting deep to the thoracolumbar fascia. Lateral to it lie the longissimus thoracis and, farther laterally, the iliocostalis, while the trapezius and latissimus dorsi have been reflected or removed to expose the deeper paraspinal architecture. Superiorly, slips approach the upper thoracic region near the rib cage, and inferolaterally the external oblique becomes visible at the flank as the back transitions toward the abdominal wall. Midline bony landmarks, including thoracic spinous processes and portions of the ribs, anchor the muscle’s orientation and attachments. Emphasis on spinalis thoracis matters because this medial erector spinae component is easily overlooked in broad “back muscle” plates, yet it becomes clinically relevant when you are teaching segmental extension mechanics or correlating paraspinal findings on imaging. Deep paraspinal injury, postoperative denervation, and chronic myofascial pain syndromes often localize near the spinous processes, where spinalis and multifidus sit in layered proximity, and this relationship helps explain why tenderness or edema can track along the midline.
