Seen from a posterior surface anatomy perspective, the semispinalis thoracis occupies the deep intrinsic back layer, running superomedially from transverse processes of the upper thoracic vertebrae toward the spinous processes several segments above, immediately medial to the longissimus thoracis and deep to the spinalis and overlying thoracolumbar fascia. Superficially, the trapezius spans from the nuchal region and thoracic midline to the scapular spine and acromion, with the posterior deltoid capping the lateral shoulder. Inferiorly and laterally, latissimus dorsi sweeps toward the humerus, while the erector spinae columns flank the spinous processes and blend caudally with the sacral attachments; gluteus maximus, hamstrings, and the gastrocnemius-soleus complex continue the posterior kinetic chain into the lower limb. Deep extensors matter here. Clinically, semispinalis thoracis and its neighbors are frequent sources of midline and paraspinal pain after sustained flexed posture, lifting, or thoracic kyphosis, where segmental extension and contralateral rotation increase load across thoracic zygapophysial joints and their medial branches. Posterior spinal approaches and thoracic instrumentation often require layered dissection through trapezius, thoracolumbar fascia, and erector spinae to reach the laminae and spinous processes, and this view helps clarify why meticulous midline splitting and retraction can reduce denervation and postoperative atrophy of the intrinsic extensors. It also supports teaching the anatomic basis of erector spinae plane blocks, where injectate deep to erector spinae can track cranio-caudally along fascial planes adjacent to the transverse processes.
