Description
Running deep to the erector spinae mass, the semispinalis thoracis is presented as paired longitudinal muscle columns flanking the thoracic spinous processes. Fibers arise from the transverse processes in the lower thoracic and upper lumbar region and course superomedially to insert on the spinous processes of upper thoracic and lower cervical vertebrae, crossing multiple motion segments. The posterior rib cage frames the field laterally, while the vertebral column remains the central bony reference from the occiput to the pelvis. Layering is the point. Because semispinalis thoracis sits in the transversospinalis group (deep to longissimus and spinalis), it is easy to lose in broad “back extensor” diagrams, yet it is a frequent source of segmental extension and contralateral rotation mechanics that clinicians test in thoracic pain patterns. This posterior perspective supports teaching of why thoracic facet loading, sustained extension postures, and paraspinal overuse can generate deep midline or paramedian symptoms that track along spinous processes rather than along the ribs. It also maps the muscle’s multi-segment attachments that matter during posterior approaches to the cervicothoracic junction, where midline dissection and retraction can denervate or devascularize deeper paraspinal layers if planes are not respected.
