Description
Posterior cervical musculature is presented with the semispinalis cervicis exposed in the deep intrinsic layer, running superiorly and medially from the transverse processes of the upper thoracic vertebrae (classically T1 to T6) to the spinous processes of C2 to C5. Its fascicles lie deep to trapezius and splenius capitis, and typically deep to semispinalis capitis, occupying the gutter between the cervical spinous processes and the articular pillars. Superiorly the occipital bone frames the craniovertebral junction, while the scapula provides a lateral reference for the cervicothoracic transition and adjacent attachments of levator scapulae and trapezius. Semispinalis cervicis matters because it is a prime segmental extensor and contralateral rotator of the cervical spine, and its tendonous insertions on C2 (axis) and the midcervical spinous processes make it a reliable landmark when teaching the architecture of the transversospinalis group. This view supports surgical and interventional orientation in the posterior neck where midline dissection for cervical laminectomy or posterior fusion proceeds through superficial layers toward the nuchal ligament and the deep extensors, and where postoperative axial neck pain can relate to detachment or denervation of the deep paraspinal muscles.
