Description
Posterior neck anatomy is presented with the splenius cervicis traced from its broad origin on the spinous processes of the upper thoracic vertebrae to its insertions on the posterior tubercles of the transverse processes of C1 to C3 or C4. Deep to the trapezius (and typically deep to splenius capitis where present), the splenius cervicis runs superolaterally, forming part of the muscular bed lateral to the cervical spinous processes and medial to the posterior triangle region. Surrounding landmarks include the occipital bone and upper cervical vertebrae superiorly, with the scapula, clavicle, proximal humerus, and adjacent ribs giving shoulder girdle context. Orientation matters here. The splenius cervicis is a primary ipsilateral rotator and lateral flexor of the cervical spine, and this long oblique fiber direction helps you teach how paired posterior muscles generate extension while unilateral contraction turns the face toward the same side.
