Description
Shown from a posterior oblique perspective, the cranial vault and upper cervical region are framed by the superficial muscular envelope of the neck and upper back. Superiorly, the occipital squama and external occipital protuberance sit in the midline, with the superior nuchal lines curving laterally toward the mastoid region, where the sternocleidomastoid attachment becomes apparent near the superior nuchal line and mastoid process. Inferior to the occiput, the atlanto-occipital transition is implied at the suboccipital region over C1, while the trapezius spans from the nuchal ligament and spinous processes to its lateral insertion on the scapular spine and acromion. Laterally, the posterior deltoid caps the shoulder, and deeper scapular contours suggest the supraspinatus and infraspinatus fossae beneath overlying fibers. Posterior neck anatomy matters when you need reliable surface landmarks for the upper cervical spine, occipital nerve pathways, and posterior surgical exposure. Tension or hypertrophy at the trapezius and adjacent suboccipital musculature often correlates with cervicogenic headache patterns, and the greater occipital nerve typically emerges near the inferior nuchal line before ascending over the occiput. Needle placement, regional blocks, and midline approaches benefit from clear orientation to the nuchal ligament, the spinous process line, and the relationship of the occiput to C1 and C2.
