Description
Running deep to the superficial trapezius, the splenius cervicis occupies the posterolateral neck as a paired, straplike muscle that ascends from the upper thoracic midline toward the transverse processes of the upper cervical vertebrae. Its fibers course superolaterally, lying lateral to the spinous processes and nuchal ligament, and blending in the posterior cervical musculature adjacent to the splenius capitis superiorly and the semispinalis cervicis and capitis more medially. On a posterior male figure, the muscle’s position is read in relation to the scapular girdle below, with the deltoids and latissimus dorsi framing the lower field while the cervical paraspinals form a midline column. Bilateral symmetry is clear. Posterior orientation matters here because splenius cervicis is a frequent culprit in mechanical neck pain that worsens with sustained extension or ipsilateral rotation, and its trigger points can refer pain toward the posterior neck and shoulder, confusing the picture with trapezius-related symptoms. Palpation and dry needling targets differ: splenius cervicis lies deeper and more lateral than semispinalis, and it is crossed by the trapezius and the posterior border of the sternocleidomastoid at the lateral neck, so surface anatomy can mislead. Dorsal rami of cervical spinal nerves supply it. A small detail with clinical consequences. Use this illustration in gross anatomy and kinesiology teaching to differentiate the superficial trapezius from the deeper posterior cervical extensors and rotators, or in physical therapy and sports medicine materials on whiplash-associated disorders and myofascial pain patterns.
