Description
Centered on the posterior neck, the paired semispinalis capitis bellies ascend from the cervicothoracic region to a broad attachment on the occipital bone, approaching the area between the superior and inferior nuchal lines near the midline. Deep to the superficial trapezius layer (not emphasized here), the muscle fibers run superomedially from the transverse processes of the upper thoracic vertebrae and the articular processes of the lower cervical vertebrae toward the posterior skull. Inferiorly, the illustration frames the cervical spine from the atlas (C1) and axis (C2) through C7, with the upper thoracic vertebrae and rib cage beginning to appear, while the scapulae sit laterally over the posterior thorax. Posterior visualization of semispinalis capitis matters because it is the dominant extensor of the head and a major contributor to contralateral rotation and segmental stabilization across the cervical spine.
