Description
Presented in posterior anatomical position on an adult male, the superficial trapezius is highlighted across the upper thoracic back, spanning from the midline spinous processes toward the acromion and scapular spine, with the deltoid laterally and latissimus dorsi inferolaterally framing the field. Deep to this layer, the semispinalis thoracis occupies the medial paraspinal compartment, its segmented fascicles running superomedially from transverse processes of the lower thoracic vertebrae to spinous processes several levels above, immediately lateral to the spinous processes and medial to the longissimus thoracis. Scapular borders and portions of the thoracic spinous processes remain visible as key landmarks for orientation. Clear midline relationships matter. Semispinalis thoracis is frequently overlooked because it sits deep to trapezius and the erector spinae, yet it becomes clinically relevant during posterior thoracic approaches where muscle splitting and subperiosteal elevation over the lamina and facet joints can provoke postoperative paraspinal pain and spasm. This view also supports teaching the transversospinalis group as segmental stabilizers, a concept that ties directly to facet-mediated pain patterns and to the planning of thoracic medial branch blocks and radiofrequency ablation, where level counting and proximity to the zygapophyseal joints are procedural realities.
