Description
Centered in the deep posterior neck, the semispinalis cervicis is highlighted against surrounding back musculature in a posterior view of an adult male. Its muscle bellies lie medial to the splenius cervicis and deep to the upper trapezius, with short, obliquely oriented fibers running superomedially from transverse processes of the upper thoracic vertebrae (classically T1 to T6) to spinous processes of the mid cervical spine (often C2 to C5). Laterally, the levator scapulae and splenius capitis frame the cervical region, while inferiorly the thoracolumbar fascia, latissimus dorsi, and gluteal mass provide broader posterior trunk context over the rib cage and vertebral column. Fiber direction is clear. This posterior emphasis matters because semispinalis cervicis is a principal extensor and stabilizer of the cervical spine, and it is frequently encountered once the superficial layers are reflected in posterior cervical approaches. During midline exposure for cervical laminectomy or posterior fusion, the semispinalis cervicis attachments around the C2 spinous process represent a key landmark and a common site of postoperative axial neck pain if disrupted. Strain or sustained overuse of this deep extensor group also features in whiplash-associated disorder and cervicogenic headache patterns, where tenderness and trigger points can localize near the upper cervical insertions and refer pain toward the occiput.
