Posteriorly, the flexor carpi ulnaris is visible as a superficial ulnar-sided forearm muscle, arising proximally from the medial epicondyle via the common flexor tendon and from the olecranon and posterior border of the ulna, then coursing distally toward the ulnar aspect of the wrist. From this angle, its muscle belly lies medial to flexor digitorum superficialis and blends distally into a tendon that passes superficial to the flexor retinaculum to insert on the pisiform, with continuation via the pisohamate and pisometacarpal ligaments toward the hamate and base of the fifth metacarpal. Bony landmarks such as the olecranon and ulnar shaft help anchor the proximal attachment in space. Clear relationships. Clinically, this view is a practical way to teach the distinction between the two proximal heads (humeral and ulnar) and to frame the ulnar nerve’s course near the medial epicondyle and between these heads, a common site of compression in cubital tunnel syndrome. The distal tendon’s insertion onto the pisiform also ties the muscle to ulnar-sided wrist pain patterns and to pisotriquetral joint irritation after repetitive flexion and ulnar deviation. For surgeons, the proximal anatomy matters when planning medial elbow exposure or when evaluating tenderness around the epicondylar region that can mimic medial epicondylitis.
