Proximally, its humeral head relates to the medial epicondyle of the humerus, while the ulnar head tracks along the posterior border of the ulna, placing the muscle medial to flexor digitorum superficialis and anterior to the extensor carpi ulnaris mass seen on the dorsal compartment. Distally, the tendon runs superficial and ulnar, aiming toward its insertion on the pisiform with continuation via the pisohamate and pisometacarpal ligaments to the hamate and base of the fifth metacarpal.
