Posteriorly, the plantaris is identified as a small fusiform muscle belly high in the popliteal fossa, arising near the lateral supracondylar line of the femur and the oblique popliteal ligament, then tapering quickly into a long, slender tendon. That tendon descends inferiorly and medially between gastrocnemius and soleus, tracking toward the calcaneal region alongside, but distinct from, the larger tendo calcaneus (Achilles tendon). Superficial landmarks include the medial and lateral heads of gastrocnemius, with soleus forming the deeper posterior compartment contour. It is easy to miss. Recognition of plantaris anatomy matters because its thin tendon can be mistaken for a partial Achilles tear on ultrasound or MRI, and a ruptured plantaris is a classic mimic of deep vein thrombosis in patients who present with acute calf pain and swelling. Surgeons also reference this posterior relationship when harvesting the plantaris tendon as an autograft for ligament or tendon reconstruction, since its length and relative dispensability make it a practical donor in selected cases. Variation is common, including absence, which changes both imaging expectations and surgical planning. Use this posterior full body view in gross anatomy and kinesiology teaching to contrast superficial posterior compartment muscles with a small, frequently overlooked structure.
