Description
Posterior views of both lower legs frame the plantaris muscle as a slender fusiform belly arising from the lateral supracondylar region of the femur near the knee capsule, then tapering into a long tendon that descends inferomedially between gastrocnemius (superficial) and soleus (deep). The tibia sits medially and the fibula laterally, with the plantaris tendon tracking toward the medial border of the calcaneal (Achilles) tendon as it approaches the ankle. Proximally, the muscle belly lies just medial to the lateral head of gastrocnemius; distally, its tendon becomes the key landmark as it threads the posterior compartment. Small muscle, long tendon. Clinical relevance centers on how easily the plantaris can be mistaken for pathology. Acute plantaris rupture or myotendinous strain can mimic deep vein thrombosis or a medial gastrocnemius tear, classically presenting as sudden posteromedial calf pain in running and jumping sports, and this posterior relationship to gastrocnemius and soleus is what ultrasound and MRI rely on to localize the injury. The same long, thin tendon is also harvested for grafting in hand and ankle reconstruction, so recognizing its course relative to the Achilles tendon and posterior tibial neurovascular bundle matters during dissection and operative planning.
