Description
Posterior arm anatomy is rendered with the humerus centered beneath a semi-transparent envelope of soft tissue, allowing the cortical breach to stand out against the surrounding triceps brachii. The fracture disrupts the distal humerus in the region of the olecranon fossa, just proximal to the medial and lateral epicondyles, where the trochlea and capitulum would articulate with the ulna and radius on the anterior aspect. Superiorly, the proximal humerus aligns with the glenoid cavity of the scapula, preserving orientation between shoulder and elbow while keeping the injury site visually dominant. Red accenting marks the bony discontinuity and adjacent tissue response. Posterior localization matters because fractures around the olecranon fossa often change elbow mechanics immediately, limiting extension as the olecranon engages a compromised fossa and provoking intra-articular involvement when the fracture propagates toward the trochlea. Nearby neurovascular structures guide the clinical reading of this image: the ulnar nerve courses posterior to the medial epicondyle and is at risk during swelling or operative exposure, while the radial nerve transitions anteriorly around the lateral supracondylar ridge after traveling in the spiral groove more proximally. The posterior surface also frames common fixation routes, including a posterior triceps-splitting or paratricipital approach when open reduction and internal fixation is planned.
