Description
Originating in the dorsal venous network on the posterior hand, the basilic vein courses proximally along the ulnar (medial) aspect of the forearm toward the cubital fossa. From a posterior perspective, its superficial track lies in the subcutaneous tissue over the extensor compartment, running medial to the more radial (lateral) cephalic vein and joining communicating channels near the elbow that correspond to the median cubital vein. Surrounding landmarks include the dorsal forearm musculature, with the vein crossing superficial to extensor carpi ulnaris and the common extensor mass, and continuing superiorly along the medial arm toward the axilla. Bony context at the wrist and hand helps anchor its relationship to the metacarpals and ulnar border. Posterior visualization matters because it clarifies where superficial venous return parallels the extensor anatomy and where perforating veins allow flow between superficial and deep systems around the elbow. Venipuncture is usually performed from the anterior aspect, but the basilic vein remains clinically relevant for peripherally inserted central catheter placement, where its larger caliber and straighter course proximally can be advantageous, and for avoiding injury to nearby medial antebrachial cutaneous nerve branches that accompany it in the arm. Misidentifying basilic versus cephalic pathways also leads to poor cannulation strategy and higher infiltration rates.
